Keep my records up to date


Challenge: Ms. S is generally in good health. She has a number of health care providers and is actively involved in maintaining a healthy lifestyle. She has been to a number of health care providers and has not gotten records of important health information about her health-promoting and prevention activity. 


Solution: Ms. S. maintains and updates her medical records with relevant information the helps her monitor her wellness. She keeps an archive of medical records.





Getting the most from my physician contact


In preparation for seeing his physician, Mr. G’s family completes a Health Risk Assessment and reviews the personalized recommendations for early detection, immunization, and prophylaxis. They establish goals for the visit and share ahead of the visit with family and relevant providers.

Mr. G’s son completes and prints out the  Physician Visit Form which includes up to date information about Mr. G’s functioning, the medications he takes and a detailed record of treatments that he has gotten from other physicians. With all this information in place, Mr. G's family becomes a partner in the health care visit. They are able to discuss his treatment options and he receives the recommended screening and preventative services best suited for him.





Using my "Salutogenesis" action plan 



Mr. A utilizes the Optimal Living section to create his plan for Optimal Living. He learns about achieving Optimal Health through access to Salutogenesis: the process of health creation. He completes an assessment of his strengths, lifestyle questionnaires and risk and disease inventories that provide a comprehensive view of his health. Mr. A reviews his Health Ecosystem and he can develop his own plan for optimal living. He has access to information about local resources that can help him. Mr. A tracks his progress using the personalized logs.
